Temporary Clinic Registration

I request permission to participate in cross-country foxhunting and other activities of the Camargo Hunt, Inc. I acknowledge that I am fully aware that the activities of the Camargo Hunt, Inc. are inherently dangerous, both to me, any horse I use, and any property, machines or motorized vehicles I use. I realize that there is constant risk of serious injury and possible death to me, any horse I use, and any property, machines or motorized vehicles I use in these and related activities of Camargo Hunt, Inc. By participating in the activities of Camargo Hunt, Inc., I realize that I am assuming all the risks, both known and unknown, that may be associated with these activities.

WARNING: under Kentucky law, a farm animal activity sponsor, farm animal professional, or other person does not have the duty to eliminate all risks of injury of participation in farm animal activities. There are inherent risks of injury that you voluntarily accept if you participate in farm animal activities. This warning is intended to include and cover all activities of Camargo Hunt, Inc.

The foregoing release is given for the purpose, among other things, of providing a waiver under Section 2305.40 of the Ohio Revised Code and Chapter 247 of the Kentucky Equine Activity Statute.

In recognition of the fact that I am assuming all the risks, both known and unknown, that may be associated with the activities of Camargo Hunt, Inc., including the constant risk of serious injury and possible death to me, any horse I use, any property, machines or motorized vehicles I use in these activities, in exchange for being permitted to participate in these activities, I on behalf of myself and my representatives, heirs and assigns, hereby release, waive, discharge, and agree not to sue Camargo Hunt, Inc., its Joint Masters, officers, directors, huntsmen, whippers-in, agents, representatives, members, and employees in their capacities as such, and the owners and lessees of the land where injuries to me, the horse utilized by me, or the property, machines or motorized vehicles utilized by me may have occurred (hereinafter referred to as “Released Parties”) and I further agree to indemnify the Released Parties, including the payment of attorney’s fees and costs, and to hold each of the Released Parties harmless with respect to any losses or damages, injuries or death, to me, the horse utilized by me, or the property, machines or motorized vehicles utilized by me, whether caused by the Released Parties or otherwise.

It is further understood and acknowledged that I have not been influenced into entering this Release and Waiver of Liability, that I have carefully read it, that I know the contents of it, and that I am signing it as my own free act.

MEDIA RELEASE The Camargo Hunt promotes positive communications about our activities, educational and conservation efforts through outlets including but not limited to various social media and publications. By signing this waiver, it is understood that I grant permission for the use of photos and name of the person named on this waiver.
